When you first attend surgery you will be asked to complete a number of forms. It would be helpful if you could bring the following information with you for everyone who is joining the practice
- name
- address
- telephone number (if you have one)
- existing health card
You can also complete a New Patient Registration Form online here for each person joining the practice.
At the time we will make an appointment for you to see our Practice Nurse for a health check. She will ask you about your medical history and carry out a few checks, such as checking your height, weight and blood pressure. This is a routine procedure that all new patients need to have. It helps us to provide the best care for our patients.
